Disease Management Programs

The DM Program is focused on improving quality of life for individuals with chronic conditions by preventing or minimizing the effects of diseases through integrated care. DM Programs are designed to improve the health of persons with chronic conditions and reduce associated costs from avoidable complications by identifying and treating chronic conditions quickly and more effectively.

PHP addresses five areas of focus:

  • Diabetes
  • Asthma
  • Hypertension
  • Tobacco Cessation
  • Prenatal Education

Diabetes Prevention Program

One in three adults has prediabetes. Without taking action, many with prediabetes will develop type 2 diabetes, which is a leading cause of kidney, eye, and heart disease. Do you have prediabetes?

Members may qualify to enroll in a FREE Diabetes Prevention Program. The program will guide you to a healthier life by supporting key lifestyle changes, including increasing physical activity, portion control, finding a healthy weight, group support, staying motivated, and stress management.

The Diabetes Prevention Program is 16 weekly sessions, followed by 8 monthly sessions. See the Diabetes Prevention Program flyer for information on how to test your diabetes risk and enroll in this fully virtual group program.
