The MyPHP Provider Portal is Now Live!

Please review the provider registration flyer for instructions. You will need to register for a new account, even if you previously had a MyPHP portal account. Your current login credentials will not work for the new portal.

Please remember to designate one Local Administrator for your office and have that individual register for the new provider portal. Once PHP confirms this individual, they will be able to add other users from your organization, including additional Local Administrators.

Our improved MyPHP Agent and Employer portals are now live!

The new MyPHP Agent and Employer portals have launched. See the employer registration flyer for information on how to register for the employer portal. For information on how to register for the agent portal, see the agent registration flyer. You are required to create a new account, even if you have previously used the portals.

Member Reference Desk contains downloadable forms and documents for your health plan. Once you enter your group number and subscriber number, you will be able to download your benefit summary, PHP handbook, certificates of coverage, advance directives, privacy statements, pharmacy mail order forms, and prescription drug lists.
