Medicare documents and forms must be accessed through the Medicare Advantage Portal.
- Login to the
MyPHP Provider Portal
- Find the PHP Medicare Advantage Portal link located under the Office Management drop-down.
- Find the document or form you're looking for in the Forms & Resources link on the far right.
- Contact PHP Medicare at 844.529.3757 if you require assistance.
